The honest summary
Belize is the only country in Central America where English is the official language, and its Qualified Retired Persons program is built around a simple proposition: show 2,000 dollars a month of foreign income from age 40, and receive a residence status that exempts you from Belizean tax on all foreign income, allows duty-free import of household goods and a vehicle, and requires only one month of presence per year. The Belize dollar has been pegged at two to one against the U.S. dollar for decades, which removes exchange-rate risk. The trade-offs are real: Belize is a very small country of under half a million people, healthcare is limited and serious cases go to Mexico, Guatemala or the United States, and it has one of the higher homicide rates in the region, concentrated in specific areas of Belize City.

What to weigh against it
Healthcare is limited. There is no facility in Belize equipped for complex care, and serious cases are evacuated to Mérida in Mexico, to Guatemala City or to the United States. Evacuation coverage is not optional here.
Belize has one of the higher homicide rates in the hemisphere, heavily concentrated in specific districts of Belize City. The tourist and expatriate areas are far safer, but the country-level statistic is real.
The QRP does not permit employment in Belize and does not lead to permanent residency or citizenship. It is a parallel status, not a path.
Infrastructure is basic: power interruptions, unpaved roads, and limited public transport.
Prices for imported goods are high because almost everything is imported.

No, and this is the most important thing to understand about it. The QRP is a parallel status administered by the tourism board. It gives you a low-obligation base with excellent tax treatment and almost no presence requirement, but it does not accumulate toward permanent residency or citizenship. If those are your goals, the ordinary residency route is what you need, and it requires a full continuous year in Belize with no more than 14 days of absence.
How bad is the healthcare really?
Bad enough that it should shape your decision. There is no facility in Belize for complex cardiac surgery, advanced oncology or major trauma, and serious cases are evacuated to Mexico, Guatemala or the United States. Primary and routine care is fine and cheap. If you have a significant condition or expect one, either choose Corozal so Mérida is reachable by road, or choose a different country.
What is the currency situation?
Excellent, and unusually simple. The Belize dollar has been pegged at two to one against the U.S. dollar for decades and U.S. dollars circulate alongside it. The expensive mistake: translating before the document is authenticated. The apostille or legalisation certificate is itself part of what the receiving government reads, so it has to be inside the translation. Do it the other way round and you pay for the same translation twice.
