How healthcare works in Italy for a foreign resident: what the public system will and will not do for you, what private cover costs, and where the system is weak.

Italy runs a regionally administered national health service that consistently ranks among the best in the world for outcomes and is very inexpensive at the point of use. Regional variation is significant: Emilia-Romagna, Veneto, Lombardy and Tuscany run excellent systems, while parts of the south lag, which is a genuine consideration if you are moving south for the tax regime.

Access to the public system

Elective residence holders register voluntarily with the SSN through the local ASL after registering residence at the comune. The voluntary contribution for non-EU residents is now assessed on income with a substantial annual minimum, replacing the flat fee that applied for many years. Once registered you choose a medico di base, your primary care doctor, and access the system on the same terms as Italians.

What private care costs

A private specialist visit is typically 100 to 200 euros. Private insurance is available and inexpensive relative to the United States, and many residents use private care to bypass public waiting lists for diagnostics.

What the visa requires

For the visa you must show private coverage valid in Italy with at least 30,000 euros for emergency treatment and repatriation. You keep it until SSN registration is complete.

Where it is strong

Excellent hospital medicine, very low prescription costs, and a strong primary care structure through the medico di base. Life expectancy is among the highest in the world.

Where it is weak

Wide north-south variation in quality and waiting times. English is not widely spoken by public-sector doctors outside major cities and tourist areas. Bureaucracy around registration is slow.

Will Medicare cover me in Italy?

No. Original Medicare does not pay for care outside the United States except in a few narrow situations. Most retirees keep premium-free Part A, make a deliberate decision on Part B based on how often they will return to the U.S. for care, and buy local or international cover.
