Italy
Italy offers something no other European country does: a 7 percent flat tax on all foreign-source income, including pensions, for retirees who move their tax residence to a town of fewer than 20,000 people in one of eight southern regions, available for nine tax years. For an American with a substantial pension that is a decisive financial advantage, and it exists precisely to repopulate the Mezzogiorno. The route in is the elective residence visa, which requires passive income of roughly 31,000 to 32,000 euros a year for a single applicant, a registered lease or property deed before you apply, and an absolute prohibition on work of any kind, including remote work. Italy is also the country where the largest number of Americans have a citizenship claim by descent, although that route was sharply narrowed in 2025. Set against all of this is Italian bureaucracy, which is slower and more variable than anywhere else in Western Europe, and a language barrier that is real outside the major cities.
Dominican Republic
The Dominican Republic runs the quickest retirement residency in the Americas. Under Law 171-07 a pensionado with 1,500 dollars a month of pension income is fast-tracked to permanent residency, commonly in a couple of months rather than the year or more that Costa Rica or Uruguay take. The law also grants a package of exemptions: on import duty for household goods and a vehicle, on property transfer tax, on the tax on dividends and interest from Dominican sources, and partial relief on property tax. It is Caribbean, so it is warm year round, it is two and a half hours from Miami, and the north coast around SosĂşa and Cabarete has hosted a foreign community for forty years. The trade-offs are hurricane exposure, infrastructure that includes frequent power interruptions outside the resort areas, and healthcare that is good in Santo Domingo and Santiago and limited elsewhere.
The real difference
- Dominican Republic costs roughly 32 percent less per month for a couple living comfortably.
- Dominican Republic has the lower income bar: $1,500 a month against $2,816.
- Italy has materially stronger healthcare on our assessment, which matters more than any other line here if you expect to need it.
- Dominican Republic is substantially faster to residency: about 4 months against 10.
Italy or Dominican Republic: common questions
Is Italy or Dominican Republic cheaper to retire in?
Dominican Republic is cheaper. A couple living comfortably needs about $2,100 a month there, against $3,100 in the other, a difference of roughly 32 percent. That is a large enough gap to change what your income supports.
Which has the lower income requirement?
Dominican Republic, at $1,500 a month on the Pensionado Residency (Law 171-07 fast track), against $2,816 for the other. Remember that a visa threshold is what the government asks you to prove, not what you need to live on, and the two numbers are often very different.
Which is faster to get residency in?
Dominican Republic is typically faster: about 10 months for Italy and 4 months for Dominican Republic. In both cases the decision is rarely the slow part; appointment availability and document lead times are.
Do my documents need an apostille for both?
Yes. Both Italy and Dominican Republic are parties to the Hague Apostille Convention, so an apostille is the whole chain for each. Your FBI Identity History Summary still goes to the U.S. Department of State rather than a Secretary of State, because it is a federal record.
Which has better healthcare?
Italy scores higher, 8 against 5 out of 10. If you expect to need care, weight this above almost everything else on this page.
